Who are applied to the Employees Health Cards through Online the Candidates will Keep the following Documents to Scan and Upload to Health Card Enrollment .
Required Documents for Submit or Upload to Employee Health Cards Enrollment Online application
• Passport Size Photograph
• Aadhaar card or Enrollment (UID / EID) receipt
• Service Register for employees, which is having name etc with clear visible or Pension Order of Pensioners
• Disabled Certificate (if employee are disabled)

1. Log in to the www.ehf.gov.in website
2. Submit the username and get password
3. Enter the Aadhaar number or Aadhaar Enrollment
4. Select "Retrieve Details " then the Application form is Opened
5. Fill the forum with Required Details
6. Then Upload Scanned Docs Then Submit the AP Employees Health Cards online Enroll application .
After then the Candidate will Getting a confirmation message to applicant mobile . Then Print the Health Cards application from and sign and make scan copy and upload after re-login , complete the process by pressing the Submit
